During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 4/1/2026 and completed on 4/7/2026. The on-site inspection began at 12:15 PM and ended at 1:10 PM. The inspector reviewed compliance in the areas listed above. There were 13 children present (including one resident child) and 3 caregivers; the point total was 30. The inspector reviewed 4 children?s records, 1 caregiver record and 3 household members records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/14/2026. A POC submitted after this date will not appear on the public website.

The center is required to obtain a completed sworn statement for household members by the end of the 30th day of reaching the age of requirement.
Household member #2's sworn statement was overdue by 17 months.

Household members shall submit to a nationwide fingerprint search by the end of the 30th day of residence after reaching the age of requirement.
Household member #2's fingerprint results were overdue by 17 months.

Standard 8VAC20-800-170-A
The provider shall obtain from each adult household member a current Report of Tuberculosis Screening form published by the Virginia Department of Health or a form consistent with it documenting the absence of tuberculosis in a communicable form.
Household member #2's record did not contain documentation of a negative screening result.
The result was overdue by 17 months.

Standard 8VAC20-821-170-B
Any person 14 years of age or older living in the home of a licensed or registered family day home shall request a Virginia child protective services central registry check within seven days of reaching 14 years of age or beginning to live in the home.
Household member #1 did not have a central registry search requested within seven days of reaching 14 years of age. The results of the search were overdue by 13 months.
Plan of Correction: The search request was submitted and results were received 3/24/2026. Results are now on file.
