Inspector Notes

An unannounced, onsite renewal inspection was initiated on 12/10/2024 and completed on 12/12/2024. The on-site inspection began at 9:15 am and ended at 11:20 am. The inspector reviewed compliance in the areas listed above. There were 13 children present and 3 staff. The inspector reviewed 5 children?s records, 3 staff records and 2 applicant records during the on-site inspection.

The inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Providers must obtain a completed national criminal background check prior to the employee?s first day of employment.

Staff 3, who has been employed for 15 months, did not have a completed national background check that had been requested by the provider. The check on file had been requested by a different licensed provider; background checks are not transferable.
Plan of Correction: Will have her complete the check.
