Inspector Notes

An unannounced on-site renewal inspection was conducted on 03/30/2023. There were 8 children, ages 2 years to 10 years at a total of 16 points with 2 caregivers, present during the inspection. The inspector observed the following: circle time, large motor play outside. A total of 5 children?s records and 2 staff records, were reviewed. The inspector discussed the following with staff: attendance requirements, playground safety. The inspector arrived at 10:10 and departed at 11:45:00

Based on observation of the stationary playground equipment, the licensee failed to ensure all requirements were met.

Evidence:
The two large pieces of stationary equipment did not have the required six feet of perimeter between the two pieces.
Plan of Correction: The licensee during the inspection moved one piece to ensure that both pieces had the six foot perimeter.
