During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previousinspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If
there were any serious injuries or fatalities related to a violation, the details will be included in the
description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 08/05/2025 and completed on 08/05/2025. The on-site inspection began at 8:25amand ended at 10:16 am. The inspector reviewed compliance in the areas listed above. There were 26 children present and 5 staff. The
inspector reviewed 10 children's records and 5 staff records on site. This inspection included document review, tour of the facility, interviews,observations and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation
cited on the violation notice. Specify how the violation will be or has been corrected. Submit your
POC within five business days from today, which will be the close of business on 08/18/2025. A POC submitted after this date will not appear on the public website.

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Outside on the playground there was a broken grey truck with no wheel, broken toy push lawn mower with a broken wheel , broken plastic clear storage bin with sharp edge and a bolt raised on the border of the playground causing tripping hazard or entanglement..

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include the following all required.

Incident report #1 was missing time parent was notified and Incident report #4 was missing Any future action to prevent recurrence of the injury and date and time parent was notified.
