Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 8/21/2024 and completed on 8/21/2024. The on-site inspection began at 2:20pm and ended at 3:30pm. The inspector reviewed compliance in the areas listed above. There were 12 children present and 2 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-50-B
Within seven business days of the child's first day of attendance at the family day home, the provider shall obtain verification of the identity and age of the child from the parent.

Child #1, who had been enrolled for more than seven days, did not have verification of identity and age.
Plan of Correction: The caregiver will obtain proof of identification for the child.
Standard 8VAC20-800-90-C
The family day home shall obtain documentation of additional immunizations
for a child who is not exempt from the immunization requirements once every six months for children under the age of two years.

There was not documentation of additional immunizations for Child #2 once every six months while they were under the age of two years.
Plan of Correction: The caregiver will obtain documentation of updated immunization for the child.
