Inspector Notes
An unannounced monitoring inspection was conducted on 03/25/2024 from 9:15 a.m. to 12:25 p.m. with the Director. There were 66 children in care, ranging in age from three months to five-years-old, supervised by 14 staff. The children were observed in circle time and going outside. Five child records and five staff records were reviewed. Eight medications and authorization forms were reviewed, and the center has one staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The first aid kit, flashlight, and battery-operated radio were observed. The most recent Fire Inspection on file was dated 02/09/23. Lunch served was chicken sandwich, pears, green beans, and milk. Areas of non-compliance are identified in the violation notice. Violations
4Evidence: The record for Staff #4, with a start date of 2/26/24, did not contain documentation of completed orientation training at the time of inspection.
Evidence: On the three-year and up playground warped boards on the fence were observed along with nail pops near the ground posing an impalement and scratch hazard. The playground had 1- ? to 3-inch gaps in the seam of the artificial grass in the area where children play, posing a trip hazard.
Evidence: The record for Staff #4, with a start date of 2/26/24, did not contain documentation of two references at the time of inspection.
Evidence:
1. In the three-year class there were 16 children present, the written record reflected 15.
2. The toddler class had no written record of attendance when they were at the bounce house. Staff stated that the attendance was in their classroom and that they did not bring the attendance with them.
