During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 06/04/25 and completed on 06/05/25, as a part of the licensure period. The onsite inspection began at 10:50am and ended at 2:10pm. The inspector reviewed compliance in the areas listed above. There were 68 children
present and 13 staff. The inspector reviewed six children?s records and six staff records on-site. Additional documentation was reviewed electronically on 06/05/25. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 07/07/25. A POC submitted after this date will not appear on the public website.
Violations
6Staff #5, employed for more than a month, did not have documentation of requesting a central registry check from three states or documentation of requesting a criminal history check from two states they resided in the preceding five years.
The TB in the record of Staff #4, who has been employed for four months, was completed more than two months after the date of employment.
Staff #2's updated screening was due 43 days ago. The updated screening has not yet been completed.
Staff #2 completed the DOE-sponsored orientation 29 days late.
Two staff were observed supervising 24 children from approximately 10:45am-11:30am.
The references for Staff #4, who has been employed for four months, were obtained a day late.
