Inspector Notes

This inspection was conducted by licensing staff using and alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on August 12, 2021 and concluded on September 1, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 55 children present directly supervised by 13 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. The inspector reviewed on 5 children records and 5 staff records.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on staff records review, 2 out 5 records, the center did not obtain documentation that at least every two years rom the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department.
Evidence:
1. Staff #2 (date of employment 6/07/2021) the center did not have documentation of expired tuberculosis screening or a current screening in record available for review during the inspection.
2. Staff #5 (date of employment 6/2021), tuberculosis screening expired on 1/11/2019. The center did not have documentation of a current screening in record available for review during the inspection.
