During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 5/28/2025 and completed on 5/29/2025 in response to a complaint received by the licensing office on 5/22/2025 relating to allegations being out of ratio. This inspection included; document review, tour of the facility, interviews and observations.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/11/25. A POC submitted after this date will not appear on the public website.

The staff-to-children ratios are required whenever children are in care.
For 3 year olds up to school age - ratio 1:10 (staff: children).
On 5/28/25 at approximately 11:15 AM, the Pre-K classroom was observe to be out of ratio with 11 children and 1 staff person present.

When children are in ongoing mixed age groups, the staff-to-children ratio and group size applicable to the youngest child in the group shall apply to the entire group.

The Pre-K classroom (3 years old to School age) has been combined with the School Age classroom after school and during the week of 4/14/2025 - 4/18/2025. The appropriate ratio for this combined group was 1 staff to 10 children.
*Center Administration confirmed the classrooms were combined and operating at a 1:18 ratio,
* Review of attendance sheets for the week of 4/14/2025 confirmed that the room was out of ratio on multiple days.
