Inspector Notes
An unannounced inspection was conducted on 3/30/23 from1:00pm to 2:50pm in response to a complaint received by the department on 3/12/23. The complaint was related to infants sleeping in equipment not intended for sleeping, and the staff-to-child ratios not being maintained in the infant classroom. During the inspection other areas of the licensing standards were also reviewed to assess compliance. Classroom observations were made, and 4 staff members were interviewed. Children were observed sleeping during nap time. 4 children?s records, 4 staff records, attendance and emergency drill records were reviewed. The evidence gathered during the investigation supported the allegations of non-compliance with standards and violations were issued. Violations
6Evidence: Two nap mat covers with built in pillows for children were torn. The soft stuffing in the torn hole of the pillow and corners was exposed, posing a choking hazard.
Evidence:
1. On 3/30/23, four staff members were working in the Infant classroom.
2. Staff # 1 stated that Staff # 2 is the program lead for the infant classroom.
3. There were three assistant teachers (Staff # 3, Staff # 4 and Staff # 5) working in the classroom, none of whom were qualified to be a program lead.
Evidence:
1. At approximately 1:10 pm, there were 14 children (ages 8 months to 16 months) in the Crawlers room with two staff members.
2. A third staff member arrived in the classroom at approximately 1:13pm.
3. The classroom was observed to be over ratio from approximately 1:10pm to 1:12pm.
Evidence:
1. Child # 5 (4 months old) was observed sleeping in a car seat.
2. Staff # 2 stated that Child # 5 had been in the car seat for approximately 15 minutes, and that he was placed in the car seat so he could fall asleep.
3. Child # 5 was not moved to his assigned and available crib once he had fallen asleep.
4. Child # 1 (10 months old), Child # 2 (8 months old), Child # 3 (11 months old) and Child # 4 (10 months old) were observed sleeping on rest mats and covers.
Evidence: In the Older Infant room (12-16 months), 4 children under the age of two were observed sleeping on rest mats with a cover that had built in pillows.
Evidence: Two nap mats in the older infant classroom and one nap mat in the 2?s-3?s classroom were not labeled with the children?s names.
