Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.

A monitoring inspection was initiated on 01/31/2022 and concluded on 02/01/2022. The provider was contacted by telephone and a virtual inspection was conducted. There were 8 children present with the provider and one caregiver supervising. The inspector reviewed compliance in the areas of administration, personnel, household members, caregiver training, physical environment and equipment, care of children, preventing spread of disease, emergencies, and nutrition. A total of 2 child records and 2 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the provider did not obtain documentation that a child has been adequately immunized before the first date of attendance for one child.

Evidence:
Child #1 began attending the family day home on 11/15/2021. The immunizations in the record for Child #1 were dated 12/13/2021.
Plan of Correction: Documentation of immunizations were obtained and placed in the record for Child #1.
