Inspector Notes

An unannounced monitoring inspection was conducted today from 9:45am-11:00am. There were 11 children directly supervised by the provider and one assistant caregiver. The home, 2 caregiver records, 5 children?s records, 1 household member?s records, and evacuation drills were inspected. Children were observed playing freely and learning about animals during circle time. Handwashing procedures were also observed. Areas of non-compliance are identified in the Violation Notice.

Based on record review, the provider did not obtain documentation of a physical examination within 30 days after one child's first day of attendance.

Evidence:
1. Child #1 began attending the family day home on 3/18/2024.
2. The record for Child #1 did not contain documentation of a physical exam.
Plan of Correction: The provider will request the documentation from the parent.

Based on record review, the provider did not obtain documentation that one child has been adequately immunized before the child may attend the family day home.

Evidence:
1. The record for Child #1 (start date 3/18/2024) did not contain documentation of immunizations.
