Inspector Notes

The following inspection took place between the hours of 10 am through 3:30 pm. Staff files were reviewed by a representative from the Commonwealth of Virginia, Department of Education, Office of Child Care Health and Safety (Fairfax) and through the County of Loudoun, Department of Parks, Recreation, and Community Services? main office located in Leesburg, Virginia. A total of 20 staff files were reviewed for this facility.

Information gathered during the inspection determined that there were (3) non-compliances with the applicable standards, and code. These violations were documented on the Violation Notice issued to the center.

Standard 8VAC20-770-40-D-1-a
Based on review, 2 of 20 staff files indicated that it has been more than five years since the date of the most recent central registry (CPS) findings.

Evidence -
1. On the date of inspection (06/27/2023), the following staff had expired CPS findings on file: Staff #2 (dated - 09/28/2017), and Staff #8 (dated - 02/02/2018).
2. This non-compliance was observed after a review of each staff person's digital file.
Plan of Correction: Will follow-up with CDC to place missing documentation within our internal system. If needed, new CPS requests shall be sent off within 10 days.
Standard 8VAC20-780-160-C
Based on review, 1 of 20 staff files indicated that it has been more than 2 years from the date of last tuberculosis (TB) documentation kept on file.

Evidence -
1. On the date of inspection (06/27/2023), the TB documentation on file for Staff #20 was dated - 03/16/2021.
2. A more current TB documentation was not available at the time of inspection.
Plan of Correction: Will follow-up with CDC to place missing documentation within our internal system.
Standard 8VAC20-780-240-A
Based on review, 1 of 20 staff files indicated that the Virginia Department of Education sponsored orientation course was not completed within 90 calendar days of employment.

Evidence -
1. On the date of inspection (06/27/2023), documentation of the VA Pre-service 10 hour training was not available for review for Staff #7.
2. The start date for Staff #7 was listed as 03/31/2022.
Plan of Correction: Will follow-up with CDC to place missing documentation within our internal system.
