During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on and completed on 04/22/2025. The on-site inspection began at 9 am and ended at 12:30 pm. The inspector reviewed compliance in the areas listed above. There were (188) children present and (26) staff. The inspector reviewed (10) children?s records and (4) staff records on-site on 04/22/2025.

This inspection included: document review (i.e. Files, injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms); tour of the facility; interviews; and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Hazardous substances must be in a locked place that prevents access by children.

In Room #12, two containers of disinfectant wipes were in an upper unlocked cabinet, out of reach of children. In Room #6, one container of disinfectant wipes was observed in the room's bathroom, out in the open, in an area accessible to the children in care.

Long-term prescription drug use and over-the- counter medication may be allowed with written authorization from the child's physician and parent.

Medication on site for Child #6 was missing a signature for the child's parent on the medication authorization form.
Plan of Correction: Parent will be contacted at pick-up in order to sign the medication authorization form. The signed form will be placed on file.
