Inspector Notes

An inspection was initiated and completed on 02/26/2026 in response to a complaint received by the licensing office on 02/18/2026 relating to an allegation involving standard 8 VAC 20-780-410. The inspector reviewed (1) child's records and (1) staff record on-site on 02/26/2026. In addition, 5 individuals were also interviewed on the date of inspection.

This inspection included document review (i.e. Files, and written statements), tour of the facility, interviews, and observations (video recordings).

The preponderance of evidence gathered during the investigation does not support this allegation. Therefore, the complaint is determined to be Not-Valid. The information gathered during the inspection determined no violations with an applicable standard or law. No violations were issued.
