An unannounced inspection was conducted to investigate a complaint related supervision, and care of the children in care at the center. The investigation was initiated on December 6, 2023, with the Director beginning at 12:41pm and ending at 1:37pm. Center documentation was reviewed and staff interviews were conducted. Classroom observations were conducted. It was determined that the complaint regarding supervision was valid.

Based on staff interviews and documentation review, staff failed to ensure that all children under the age of 10 years old were within actual sight and sound supervision of staff. Evidence: On September 21, 2023, staff A, staff B, and staff C left a 2-year-old child outside on the enclosed play area unsupervised for approximately three minutes. On October 26, 2023, staff A and staff D left a 3-year-old child unsupervised in the classroom?s bathroom unsupervised for approximately 5 minutes. November 15, 2023, staff E left a 3-year-old child unsupervised in a classroom?s bathroom for approximately 3 minutes.

Based on staff interviews and documentation review, staff failed to follow the center's own policies and procedures as required by the standards. Evidence: The center's Face to Name policy for accounting for children at all times requires staff to compare the children's faces to the name listed on the written attendance during transitions from different areas of the facility. On September 21, 2023, Staff A, staff B, staff C did not complete a name to face check of all the children after transitioning from the outside play area to the classroom. Child A was left unsupervised on the outside play area for approximately three minute as the class entered the building. On October 26, 2023, Staff A and staff D did not complete a name to face after two children transitioned from the classroom after diapering and using the restroom. Child B was left unsupervised in the classroom bathroom for approximately 5 minutes.
