Inspector Notes

An unannounced monitoring inspection was conducted today between the hours of 10:30am-11:55am. There were 12 children directly supervised by 3 caregivers. The home, 3 caregiver records, 3 children?s records, evacuation drills, and emergency supplies, were inspected. Children were observed playing freely, exercising, and during story time. Areas of non-compliance are identified in the Violation Notice.

Based on record review, the provider did not document the method of verification of proof of identity for 3 of 3 children reviewed.

Evidence:
1. The records for Child #1 (start date 8/8/22) , Child #2 (start date 8/29/22), and Child #3 (start date 9/12/22) did not contain the date the proof of identity was viewed and the name of the person viewing the document.
Plan of Correction: Corrected during inspection.
