A complaint investigation was initiated on 5/18/23 from a report received by the Fairfax Licensing Office on 5/12/23 regarding physical plant and operational responsibilities. The information gathered during the investigation does not support the allegation, so the complaint is determined to be ?not valid.? Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice. Based on record review and caregiver interview, the family day home did not keep a written record of attendance for each child.

Evidence:
1. The provider stated Child #1 and Child #2 had been attending the Family Day Home since 5/8/2023. Child #1 and Child #2 were not listed on the attendance records for the family day home.
Plan of Correction: All children present will be documented on the attendance record.

Based on record review and caregiver interview, the provider did not maintain an up to date record for 2 enrolled children.

Evidence:
1. The provider stated Child #1 and Child #2 had been attending the Family Day Home since 5/8/2023.
2. The provider did not have a record for Child #1 or Child #2.
Plan of Correction: The provider will obtain all required documents for Child #1 and 2.
