Inspector Notes

An unannounced renewal inspection was conducted on 10/19/2023 between the hours of 10:00 am to 12:30 pm. There were 10 children present directly supervised by the provider and 2 assistant caregivers. The home, 7 caregiver/household member records, 3 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed dancing, singing their abc?s and nursery rhymes, and having snack. Handwashing, cleaning and sanitizing were also observed. Areas of non-compliance are identified in the Violation Notice.

Based on record review, the provider did not obtain a current Report of Tuberculosis Screening form every two years from the date of the first screening for 3 household members.

Evidence:
1. The most recent report of tuberculosis screening in the record for Household Member #1 was dated 8/23/21
2. The most recent report of tuberculosis screening in the record for Household Member #2 was dated8/13/21
3. The most recent report of tuberculosis screening in the record for Household Member #3 was dated 8/4/21

Based on record review, 2 of 3 children's records did not contain all required information.

Evidence:
1. The record for Child #1 did not contain documentation of the name, address and telephone number for one parent's place of employment
2. The record for Child #3 did not contain documentation of the address ro telephone number for one parent's place of employment.
Plan of Correction: I will ask the parents for the information.

Based on record review, the provider did not obtain documentation that one cild has been adequately immunized before the child's first day of attendance.

Evidence:
1. Child #3 began attending the family day home on 9/20/2023. The documentation of immunizations in the record for Child #3 were dated 10/02/2023.
Plan of Correction: We will get the immunizations from the parents before the first day.
Standard 8VAC20-800-920
Based on observation, the menu for the current one week period did not contain all required components.

Evidence:
1. The menu did not list substituted foods.
2. The menu listed cheerios and milk to be served during snack, children were served apples, crackers, and milk.
