Inspector Notes

An unannounced monitoring inspection was conducted today from 12:45pm-2:20pm. There were 11 children directly supervised by the provider and 1 assistant caregiver. The home, 3 caregiver records, 3 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed during naptime and playing freely. Areas of non-compliance are identified in the Violation Notice.

Standard 22.1-289.035-B-2
Based on record review, the provider did not obtain a fingerprint-based criminal history record check prior to the first date of employment for 1 caregiver.

Evidence:
1. Caregiver #3 began employment in 2/2024.
2. The fingerprint check results in the record for Caregiver #3 were dated 4/3/2024.
Plan of Correction: Fingerprint checks will be obtained before the first day for new caregivers.
Standard 8VAC20-800-170-B
Based on record review, the provider did not obtain a tuberculosis screening within the previous 30 days from the date of employment for one caregiver.

Evidence:
1. Caregiver #3 began employment in 2/2024. The tuberculosis screening in the record for Caregiver #3 was dated 3/1/2023, more than 30 days prior to the first date of employment.
Plan of Correction: Tuberculosis screenings will be obtained within 30 days before the first day for new caregivers.

Based on record review, the provider did not obtain a current report of tuberculosis screening every two years for 1 caregiver.

1. The most recent report of tuberculosis screening in the record for Caregiver #2 was dated 1/14/2022.
