During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 02/18/2025 , as a part of the licensure period. The on-site inspection began at 10:30a.m. and ended at 12:15p.m. The inspector reviewed compliance in the areas listed above. There were 54 children present and 12 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-440-N
Toys or objects hung over an infant in a crib and crib gyms that are strung across the crib may not be used for infants over five months of age.

A crib mobile was hung over an infant's crib who is seven months of age.
Plan of Correction: This mobile was removed immediately.
Standard 8VAC20-780-510-E
The center's procedures for administering medication shall include duration of the parent's authorization for medication, provided that it shall expire or be renewed after 10 work days. Long-term prescription drug use and over-the- counter medication may be allowed with written authorization from the child's physician and parent.

A medication authorization for child #3 has not been renewed for almost a year.
Plan of Correction: The action plan and authorization form will be given to the parent to request that the physician complete both forms.
Standard 8VAC20-780-510-H
Nonprescription medication shall be in the original container with the direction label attached.

An over the counter medication for Child #3 was not in the original container and did not have a direction label attached.
Plan of Correction: This medication will be returned to the parent today. If the parent would like to bring new medication, it must be in the original box.
Standard 8VAC20-780-60-A
Repeat
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain phone numbers and addresses for 2 emergency contacts.
Child #1 and #2's record did not contain addresses for 2 emergency contacts. Child #4's record did not contain phone numbers for 2 emergency contacts.
Plan of Correction: Complete emergency contacts with names, addresses and phone numbers will be required from the parents. Attendance will be questioned if information is not provided.
Standard 8VAC20-780-60-A-8
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information: a written care plan for each child with a diagnosed food allergy, to include instructions from a physician.

The written care plan found for Child #3 was not completed by a physician.
Plan of Correction: The action plan and authorization form will be given to the parent to request that the physician complete both forms.
