Inspector Notes

A monitoring inspection was initiated on September 29, 2023 from 8:05am - 10:00am. There were 29 children with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and two staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the licensee did not ensure the center obtain documentation that each child has recieved immunization before child can attend the center.

Evidence: The center did not obtain Child #1 and Child #2 immunization record prior to attending the program.
Plan of Correction: All children files will be updated to include physicals.
Standard 8VAC20-780-140-B
Based on record review, the licensee did not ensure each child have a physical examination prior to attendance or with 30days from start date.

Evidence:
Child # 2 start date 08/28/2023. No physical obtained by the center.
Plan of Correction: All children files will be updated to include immunization forms.
