During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on August 25, 2025 and completed on August 26, 2025. The on-site inspection began at 3:26 pm and ended at 5:37 pm. The inspector reviewed compliance in the areas listed above. There were 25 children present and 5 staff including administrative staff. The inspector reviewed 10 children's records and 5 staff records on site. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 2, 2025. A POC submitted after this date will not appear on the public website.

4The findings from the most recent inspection, dated 02/11/2025, were not posted in a conspicuous place on the premises.
The sworn statement for Staff #4, who is employed for 2 months and currently working, was not found in the record.
Staff #5 updated screenings have not yet been completed.
The record for Child #10, enrolled and present at the center, was missing the following required components: Name, nickname (if any), sex, and birth date of the child; Name, home address, and home phone number of each parent who has custody; When applicable, work phone number and place of employment of each parent who has custody; Name and phone number of child's physician; Name, address, and phone number of two designated people to call in an
emergency if a parent cannot be reached; Names of persons authorized to pick up the child; Written agreements between the parent and the center as required; First and last dates of attendance.
