Inspector Notes

A monitoring inspection was conducted on 5/19/2022. There were 108 children in care with a total of 16 staff assigned to their direct supervision. The children were observed taking a walk, playing on the playground, creating art projects, and engaging in circle time discussions. The Inspector reviewed compliance in the areas of administrative records, emergency preparedness, children and staff records, background checks, the physical plant, and nutrition.

Based on a review of administrative documents, it was determined that the injury prevention procedures were not updated at least annually.
Evidence:
The annual review of the injury prevention plan was most recently documented March, 2021.
Plan of Correction: The injury prevention plan was completed, but not documented. I will update the documentation.
Standard 8VAC20-780-60-A
Based on a review of children's records, it was determined that one record is not complete.
Evidence:
Child record B does not include: each parent's employment contact information; the address for each emergency contact person.
Plan of Correction: The parent will be asked for the missing information.
