Inspector Notes

An unannounced monitoring inspection was conducted on May 28, 2024, from approximately 10:08 am to 1:25pm. Administration, physical plant, staffing and supervision, programming and nutrition were inspected. The sample of 5 staff and 12 children?s records were reviewed. The children were observed playing dress-up, painting, music & movement, Circletime and getting ready to go outdoors.

Standard 22.1-289.011-F
Based on observation, the last violation notice and inspection summary documents required by the Superintendent to be posted were not posted in a conspicuous place on the licensed premises.
Evidence:
The findings of the most recent inspection of the facility were not posted for the inspection dated 11/27/2023.
Plan of Correction: The previous inspection is posted Corrected on the immediately.
Standard 8VAC20-780-270-A
Based on observation, the center did not ensure areas and equipment of the center inside were maintained in a safe and operable condition.
Evidence:
There is a broken handle on the last lower cabinet near the back of the room by the closet in Room #218.
Plan of Correction: The Center Manager fixed the broken handle of the lower cabinet.
Standard 8VAC20-780-280-B
Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
Shaving cream and hazardous materials (bleach and Clorox disinfectant spray) were in the unlocked bottom cabinet.in Room 223.
Plan of Correction: The teachers immediately placed the shaving cream and Hazardous substances in a locked place.
Standard 8VAC20-780-280-G
Based on observation, the center did not ensure hazardous substances were kept in original containers, the substitute containers clearly indicated with their contents and not resemble food or beverage containers.
Evidence:
Green soap was stored in a plastic water bottle and not labeled in Room #309.
Plan of Correction: Asked the parents to complete the missing information.
Standard 8VAC20-780-60-A
Based on record review, the center did not ensure that each child has a record that contains all required information.
Evidence:
1. Child #10's file was missing parent work information.
2. Child #3's file (start date 8/8/19) was missing the annual parent confirmation that the required information in the child's record was up to date. It was last updated on 7/30/22.
Plan of Correction: Asked the parents to complete the missing information.
