Inspector Notes
An unannounced monitoring inspection was conducted on April 4, 2023, from 9:45 a.m. to 11:15 a.m. There were 95 children present, ranging in ages from six months to 12 years old with 18 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision and programming. Two staff records were fully reviewed, and 20 staff records were reviewed partially for required updated documentation regarding background checks, tuberculosis screenings, CPR and First Aid certifications, trainings and qualifications.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

5Based on review of records, the center failed to ensure that background checks were repeated every five years.
Evidence:
1. Staff #1?s Central Registry Results on file were dated 9/27/2017. There was no updated Central Registry Results on file.
2. Staff # 17 confirmed that the Central Registry background check for Staff #1 had not been repeated every five years.
Evidence:
1. Staff #21?s start date was 6/22/22. Staff # 21?s criminal history check on file was dated 3/17/2023.
2. Staff #17 confirmed that Staff #21?s criminal history record check was not completed before the first day of employment.
Evidence:
1. Staff #21?s start date was 6/22/22. Staff # 21?s record did not contain evidence of a TB screening.
2. Staff #21 was observed in the school age classroom.
3. Staff #17 confirmed that Staff #21 did not submit a negative TB screening at the time of employment and prior to coming into contact with children.

2. Staff #17 confirmed that Staff #21 did not complete the completed The VDOE sponsored course within 90 days of employment.
Evidence:
1. The inspector observed 37 children on the playground. Staff #6, #11, #14 and #22, confirmed that there were 37 children on the playground at the time of the inspection.
2. Measurements of the outdoor play area taken by the department on 8/15/2008 demonstrate that the maximum number of children permitted on the outdoor play area at one time is 29.
