Inspector Notes

An unannounced monitoring inspection was conducted on 06/16/2023 from 12:45 p.m. to 2:15 p.m. There were 86 children present, ranging in ages from 8 months old to 11 years old with 15 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision and programming. Six staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

REPEAT and SYSTEMIC
Based on observation, the center failed to ensure that areas and equipment of the center, Inside and outside, were maintained In a clean, safe and operable condition.
Evidence:
1. The following was observed at the center on 06/06/2023: Small debris and trash located in and around the toddler play area (See CDC-1106278(06-16-2023)P1.jpg, CDC-1106278(06-16-2023)P2.jpg), in and around the basketball/gravel play area (See CDC-1106278(06-16-2023)P3.jpg, CDC-1106278(06-16-2023)P4.jpg), in and around the large play area (See CDC-1106278(06-16-2023)P5.jpg), and throughout the parking lot and outdoor walk ways (See CDC-1106278(06-16-2023)P6.jpg).
2. Staff #6 confirmed the presence of the small debris and trash in and around the toddler play area, in and around the basketball/gravel area, in and around the large play area and throughout the parking lots and outdoor walkways.
Plan of Correction: Staff checks outdoor areas before children go outside. Administration spoke with U-Haul business about trash that blows over and spoke with parents about not throwing out trash.
