Inspector Notes
An unannounced monitoring inspection was conducted. The focus of the inspection was to determine (i) whether the provider had corrected or is in the process of correcting previously cited violations in the areas of standards or laws referenced above, and (ii) to determine if the provider remains in compliance with all provisions of the Consent Agreement, effective July 13, 2023, that was made between the Department and the provider in lieu of the enforcement action stated in the Notice of Summary Suspension and Statement of Facts issued to the provider June 23, 2023. This inspection yielded no violations of applicable laws or regulations.

REPEAT VIOLATION
Based on interview and review of records, the center failed to prepare a document containing the required information to be kept in vehicles that centers use to transport children.
Evidence:
1. The document kept on the vehicle used to transport children was missing potential shelters, hospitals and evacuation routes.
2. Staff #1 confirmed that the required items were missing from the document.
