Inspector Notes
An unannounced monitoring inspection was conducted on 10/23/2023 from 1:40 p.m. to 2:45 p.m. There were 56 children present, ranging in ages from 3 months to five years old with 13 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Three staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations
4Systemic Violation
Based on review of records and interview, the center failed to repeat background checks every five years.
Evidence:
1. Staff #3?s criminal history background check on file was dated 09/28/2018.
2. Staff #4 confirmed that the criminal history background check had not been repeated every five years.
Systemic Violation
Based on observation the center failed to ensure that areas and equipment of the center are maintained in a clean, safe and operable condition.
Evidence:
1. In the two-year-old classroom, one highchair was observed to have dried food on the tray, the seat and sides of the highchair; crumbs and debris in the crevices; and missing a seatbelt.
Based on observation and interview, the center failed to maintain the required staff: children ratio of 1:8 for children two years old.
Evidence:
1. 11 children and two staff were observed in the two-year-old classroom. Staff #1 left the classroom leaving Staff #2 alone will 11 children.
2. Staff #1 confirm that they had left Staff #2 alone with 11 children.
3. Staff #1 and Staff #2 confirmed that the age of the youngest child in the group was two years old.
Evidence:
1. In the toddler classroom, three children were observed in highchairs and the protective belts were not fastened.
