Inspector Notes

An unannounced renewal inspection was conducted on 03/04/2024 from 10:30 a.m. to 1:30 p.m. There were 22 children present, ranging in ages from 11 weeks to five years old with nine staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and six staff records were reviewed.

The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.

Standard 8VAC20-780-130-A
Based on review of documentation and interview, the center failed to obtained documentation of additional immunizations once every six months for children under the age of two.
Evidence:
1. Child #3?s date of birth was 01/13/2022. Child #3?s immunizations on file were dated 04/22/2022, 08/22/2023 and 02/08/2024.
2. Staff #2 confirmed that immunizations had not been obtained every six months for Child #3.
Plan of Correction: Staff will make sure children under 2 will obtain immunization records ever 6 months
Standard 8VAC20-780-150-B
Based on review of records and interview, the center failed to ensure that immunization records are signed by a physician, his designee, or an official of the local health department.
Evidence:
1. Child #3?s immunization record dated 02/08/2024 did not contain a signature from a physician, his designee or an official of the local health department.
2. Staff #1 and #2 confirmed that the immunization record did not have the required signature.
Plan of Correction: Staff will make sure immunization records are signed by physician.
