Inspector Notes

An unannounced, on-site inspection was initiated on May 27, 2025 completed on May 27, 2025 in response to a complaint received by the licensing office on May 19, 2025 relating to allegations in the areas of Administration , Physical Plant, Staffing and Supervision and Special Care Provisions and Emergencies. This inspection included document review, tour of the facility, interviews and observation.

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on June 5, 2025. A POC submitted after this date will not appear on the public website.

REPEAT VIOLATION
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
The playground turf contained a proud nail which caused an injury to a child on April 17, 2025 that required stitches.

Standard 8VAC20-780-80-B-2
The center shall inform the department?s representative as soon as practically possible but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.
On April 17, 2025, Child #1 slipped on the playground, cut their knee on a nail from the turf, which required emergency services to be contacted and four stitches. This injury was not reported to the department.
