Inspector Notes
An unannounced, on-site inspection was initiated on May 27, 2025 completed on June 9, 2025 in response to a complaint received by the licensing office on May 22, 2025 relating to allegations of Staff Qualifications and Training, Physical Plant, and Staffing and Supervision. The inspector reviewed one staff records on-site. This inspection included document review, tour of the facility, interviews and observation.
The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
