Inspector Notes

An unannounced monitoring inspection was initiated and completed on 5/31/2022 between the hours of 10:35am-2:00pm. There were 81 children present with 15 staff supervising. Children were observed playing outside, completing an American flag art project, painting, and playing with blocks.

A total of 4 children records and 6 staff/ agent records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Based on record review, the center did not ensure each child has received the immunizations required before the child can attend the center.

Evidence:
1. Documentation in the record for Child #2 (start date 9/27/2021) indicated the child was not adequately immunized, but would receive the remaining immunizations "within the next 90 days". There was no documentation in the record that the remaining immunizations were obtained.
Plan of Correction: Corrected during inspection.

Based on record review, the center did not request at least annually, parent confirmation that the required information in the child's record is up to date.
