Inspector Notes

An unannounced monitoring inspection was conducted today between the hours of 10:10am and 1:10pm. 72 children were observed directly supervised by 15 staff. The physical plant, 4 staff records, 5 children?s records, medications, evacuation drills, and emergency supplies were inspected. Children were observed practicing letter recognition, painting, playing with blocks, and engaging in dramatic play, There was an assortment of age appropriate materials for the children in care. Areas of non-compliance are identified in the Violation Notice.

Based on record review, 1 of 5 children's records reviewed did not contain documented confirmation that the child's record is up to date.

Evidence:
1. The record for Child #1 (start date 9/1/2020) did not contain documentation of parent confirmation that the required information in the child's record is up to date.
Plan of Correction: The form was given to the parents prior to the inspection. The center will request the form from the parents.

Based on record review, 1 of 5 children's records reviewed did not contain all required information.

Evidence:
The record for Child #2 did not contain the address for one of two designated people to call in an emergency if a parent cannot be reached.
Plan of Correction: The emergency contact address was obtained and added to the child record.

Based on record review, 2 of 5 written care plans for children with a diagnosed food allergy were not complete.

Evidence:
1. Child # 6 and Child #7 are diagnosed with a food allergy. The written care plans for Child #6 and Child #7 did not contain the parent's signature.
Plan of Correction: Parents' signatures were obtained for both allergy care plans.

Based on record review and staff interview, 1 of 4 Staff records did not contain all required information.

Evidence:
1. Staff #1 was identified by administrative staff as the lead teacher in the Frogs classroom.
2. The record for Staff #1 did not contain documentation to demonstrate that the individual possesses the education, certification, and experience required to be a lead teacher.
Plan of Correction: The center obtained documentation verifying Staff #1 was lead-qualified. The documentation was placed in the record for Staff #1.
