Inspector Notes

An unannounced monitoring inspection was conducted on 3/13/2024 between the hours of 9:10am and 2:30pm. There were 86 children directly supervised by 17 staff. The physical plant, 4 staff records, 5 children?s records, medications, evacuation drills, injury reports, and policies were inspected. Children were observed singing, dancing, playing outside, and learning about planets. Areas of non-compliance are identified in the Violation Notice.

Standard 8VAC20-780-245-M
Based on record review and staff interview, 2 of 4 staff records did not include documentation of required training.

Evidence:
1. Staff stated a training regarding the center's emergency preparedness procedures was completed with all staff on 08/16/2023.
2. The records for Staff #3 and Staff #4 did not contain documentation of the training.
2. The center was not able to provide documentation that the required annual training occurred to include the following required components:
-Name of staff;
- Training topic;
-Evidence that training on each topic required has been completed;
-Training delivery method;
- The entity or individual providing training;
-The number of training hours or credit hours received; and
-The date of training.
Plan of Correction: A training document was created to reflect the training completed on 8/16/2023.
Standard 8VAC20-780-270-A
Based on observation, areas and equipment of the center, outside, were not maintained in a safe and operable condition.

Evidence:
1. Concrete landscaping materials were laying in an area of the playground accessible to children.
2. Broken Tubing connected to the gutter, creating sharp edges, was observed outside of the building where children walk to the playground area.
Plan of Correction: The center has notified the appropriate contacts in order to have the landscaping materials removed and the tubing for the gutter replaced.

Based on record review, lockdown drills were not completed annually.

Evidence:
1. The most recent lockdown drill was documented on 12/08/2022.
Plan of Correction: A lockdown drill will be completed today.
