Inspector Notes

An unannounced renewal inspection was conducted today from 9:15am to 11:45am. There were 90 children in care directly supervised by 15 staff. The physical plant, 3 staff/agent records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing with legos, practicing cutting with scissors, and completing puzzles. Each class was completing activities related to the week's lesson "Places we live". Areas of non-compliance are identified in the Violation Notice.

Based on record review, the center did not obtain repeat background checks every five years as required for 2 of 2 Agents.

Evidence:
1. The repeat fingerprint check in the record for Agent #1 was obtained on 03/06/2023, more than 5 years after the previous results obtained on 01/20/2018.
2. The repeat central registry check results in the record for Agent #2 were obtained on 04/26/2024, more than 5 years after the previous results obtained on 08/10/2017.
3. The repeat sworn statement in the record for Agent #2 was dated 04/17/2024, more than 5 years after the previous sworn statement dated 10/12/2017.
Plan of Correction: The required background checks were obtained and filed in the record. Repeat background checks will be obtained within required time frames.
