Inspector Notes

An unannounced complaint investigation was conducted, along with Licensing Inspector Shawanda Henderson on 5/28/24 from 9:13 a.m. to 11:30 a.m. in response to a complaint received in the licensing office regarding allegations of clean, safe, and operable condition, diapering, ratios, and emergency drill procedures. Based on a review of available documentation and interviews conducted, the allegations regarding clean, safe, and operable condition, diapering, and emergency procedures were determined to be valid. The allegations regarding ratios were unable to be substantiated due to insufficient corroborating evidence. Based on observation, it was determined that not all areas of the center inside and outside were maintained in a clean, safe condition, and operable condition.

Based on observation and interview, the center did not ensure that the diapering area contained a nonabsorbent surface for diapering or changing.

Evidence: In the infant classroom, the diaper changing pad was observed to have a crack, making it absorbent.
