During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced investigation was initiated on 5/21/25 and completed on 6/12/25 in response to a complaint received by the licensing office on 5/13/25 relating to allegations of care, protection, and guidance, and injury reporting. The inspector reviewed two children?s records and two staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/14/25. A POC submitted after this date will not appear on the public website.

8VAC20-780-340-A When staff are supervising children, they shall always ensure their care, protection, and guidance.

Evidence: On 5/12/25 in the toddler classroom, ages 16-24 months, Child #1 sustained an injury to the face. Staff #1 wrote a report stating that the child fell on the carpet. It was later discovered that Staff #1 did not see the injury occur and that the injury to the child?s face was a result of being bitten. Interviews revealed that Staff #1 struggled with supervision while multi-tasking.
Plan of Correction: 1. Immediate Staff Reassignment and Dismissal: As a corrective measure, Staff #1 and Staff #2 were immediately removed from the Toddler classroom. After a thorough internal review, both staff members were dismissed due to failure to meet supervision expectations and standards outlined in 8VAC20-780-340-A.

2. Staff Replacement with Qualified Personnel: The school has since hired experienced early childhood professionals to staff the Toddler classroom. New hires hold relevant credentials and have a minimum of 10 years of direct experience in toddler classrooms, ensuring a higher level of competency in supervision, child development, and classroom management.

3. Training and Supervision Protocol Reinforcement: All staff have been re-trained on the importance of maintaining sight and sound supervision at all times. This training includes: ? Recognizing and minimizing distractions while supervising. ? Clear protocols for reporting and documenting incidents accurately. ? Immediate response procedures for any injury or behavioral incident.

4. Increased Administrative Oversight: For the next 90 days, the Director will conduct daily classroom observations and weekly supervision audits to ensure compliance with supervision standards. Feedback will be provided to staff, and corrective actions will be implemented immediately if concerns arise. 5. Incident Reporting and Communication Review: Internal policies regarding injury documentation and communication with families have been reviewed and updated. Staff have received refresher training on accurately identifying, reporting, and responding to child injuries.
