Inspector Notes

An unannounced follow-up monitoring inspection was conducted today, September 21, 2023, to check compliance with background checks. The inspector was on site from approximately 3:00 pm until 3:15 pm. There were six children present with two caregiver supervising. Two caregiver record and one household member record were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of two caregiver?s records, the provider did not obtain background checks every five years.

Evidence:

1. The record for caregiver #1 contained the results of a fingerprint national criminal record
check dated 05/30/2018, exceeding five years.
2. The record for caregiver #2 contained the results of a fingerprint national criminal record
check dated 05/30/2018, exceeding five years.
Plan of Correction: Per the provider: I requested the updated fingerprints last week but I have not received the results as of yet.

Based on review of one household member record, the provider did not obtain the results of a central registry background check within 30 days of one household member becoming 14 years of age.

Evidence:

The record of household Member #1, 14 years in age as of March 19, 2023, was missing results of a central registry background check.
Plan of Correction: Per the provider: I requested the central registry multiple times but I was requesting the wrong one. I was able to get the correct one requested and I am waiting for the results.
