Inspector Notes
An unannounced renewal inspection was conducted on 6/14/2024. The inspector was on site from approximately 9:15 am-12:49pm. There were 7 children in care, ranging in age from 1 year to 5 years with three caregivers supervising. The inspector reviewed compliance in the areas of administration, personnel, household members, physical environment and equipment, care of children, emergencies, caregiver training, preventing the spread of disease and nutrition. A total of 5 children?s records and 4 caregiver records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it within 5 business days. Please specify how the deficient practice will be or has been corrected.
Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations
3Evidence:
The record for caregiver #1 contained the results of a central registry search dated 6/13/19, exceeding 5 years. The provider acknowledged that a request to update the central registry search for caregiver #1 had not been sent in and that the most recent results for caregiver #1 exceeded 5 years.
Evidence:
In the children?s outdoor play area in the backyard, there was a large, rusted barrel. Also in the children?s outdoor play area in the backyard, there was a section of lattice under the steps that was broken and splintered. The provider acknowledged that the lattice needed to be repaired and that the barrel needed to be removed.
Evidence:
The provider did not request at least annually parent confirmation that the required information in the children?s files was up to date. Parent confirmation of the information in the files for the children was dated 4/17/23 for Child #1, 5/02/23 for Child #2, 9/02/20 for Child #4, and 3/22/22 for Child #5. The provider acknowledged that she did not have the parents confirm that the information in the children?s files was up to date annually.
