The program is required to submit to VDH and implement a plan to test potable water for lead. The program did not have evidence of a testing plan submitted to VDH and/or the program did not have evidence of the implementation of the required testing, which is 11 months overdue.
Plan of Correction: The director stated that she was not aware of this requirement. The program has now enrolled in the water testing grant program to implement a plan for water testing to be conducted. Written confirmation of the enrollment was sent to the inspector on 11/24/2025.
