Inspector Notes
An unannounced subsidy health and safety inspection was initiated and completed on September 18, 2026 in response to a complaint received by the licensing office on September 15, 2025 relating to allegations of exceeding the number of allowable children in care, physical plant and care of the children. The
inspector reviewed 3 children?s records and 2 caregiver/household records on-site.
This inspection included (include all that apply from the list below from the list below).

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable vendor requirements or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on October 16, 2025. A POC submitted after this date will not appear on the public website.

18The vendor/provider did not have a signed sworn disclosure statement for herself.

The vendor shall ensure that the family day home does not exceed the capacity of children cared for as allowed by law or regulation.
Upon arrival at 9:20 AM, there were nine children in care in the child care room with the vendor and one other adult.

The vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.

There were records provided for only three of the nine children in care at the start of the inspection.

There were no records available for staff 3, 4 and 5 who were listed on the signed parent agreements as back-up providers in the absence of the vendor/provider.

The vendor was unable to provider documentation that the vendor completed the Virginia Pre-Service Training for Child Care Staff.
I provided this the first time I just could not find it I have it now
There is a top-loading freezer without a child proof latch in the accessible dining room.
There is a front loading freezer and a front loading dryer without child proof latches in the laundry room through which children pass to access the bathroom.
There is a 3 and 1/2 foot tall fan propped and leaning over within reach of the children that could topple upon a child.
There were dangling electrical cords hanging from the wall-mounted television.
Three of five high chairs that the vendor confirmed are used for the children do not have the required seat belt systems.
A wooden tread on the stairway used by the children to access the bathroom has a missing piece that cracked off of the stair tread leaving a jagged edge.
There are plastic grocery bags and single use gloves posing suffocation hazards stored in an unlocked cabinet under the sink in the children's bathroom.
FIve bedrooms do not have child proof latches to keep children from accessing the contents of the rooms.

There is an electric fireplace in the living room that does not have a barrier or screen.
No kids are allowed in my upstair area
There are four stairs that the children climb when accessing the bathroom. These stairs do not have protective barriers or guardrails.

There are cleaning fluids and laundry soaps with warning labels on top of the washer and dryer in the hallway through which the children pass when accessing the bathroom
The following substances with warning labels were found:
- a spray can of cleaning fluid in the unlocked cabinet under the sink in the children's bathroom,
- a spray bottle of kitchen cleaner on the open counter in the kitchen,
- diaper cream on top of the stacked boxes beside the changing table.
- two containers of hand sanitizer on the children's activity table.

The vendor responded to undesirable behavior of the children by repeatedly saying "sit down", "stop", "cut it out", "I'm not going to keep talking to you", and "no". No help, support or guidance to appropriate behavior was provided. No caregiver directed activities were provided during the inspection.
The caregiver shall wash their hands with soap and running water before preparing and serving food or beverages.

At the start of the inspection, there were three partly filled infant bottles on the table in the child care room. The bottles were not dated nor labeled with a child's name.

The dog's water and food bowls on the floor of the kitchen were accessible to the children.
