Inspector Notes
A monitoring inspection was conducted on 02/21/2024 between the hours of approximately 11:15 am and 11:55 am. There were 4 children present, ranging in ages from 3 years to 5 years (8 points), with the Provider. Children were observed while playing with toys.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 1 child record and 2 staff/household member records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
4Evidence:
1) On the date of inspection, the record for the Provider did not contain documentation of having completed any of the required sixteen clock hours of annual training during the year 2023.
Evidence:
1) The record for Child #1 did not contain the following information: *Name, home address, and telephone number of each parent who has custody; name, address and telephone number of each custodial parent's place of employment; name, office address and telephone number of the child's physician; name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached; name and policy number of the child's medical insurance, and the first date of attendance.
Evidence:
1) On the date of inspection, the first aid kit did not contain a second triangular bandage.
Evidence:
1) On the date of inspection, the 2024 emergency drills log provided for review, did not have an evacuation (fire) drill documented for the month of January 2024.
