Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program.
A monitoring inspection was initiated on 3/24/2022 and concluded on 03/25/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 17 children present, ranging in ages from 5 years to 12 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of two staff records and staff interview, the facility did not obtain an out of state search of the central registry check for each staff prior within the required time frame for any state an individual has resided in the past five years. Evidence: Staff 1 (date of hire 7/19/2021) indicated on the sworn disclosure statement that she had lived outside the state of Virginia in the past five years. There was no documentation of an out of state search of the central registry check for Staff 1. The director stated she forgot about it.
Plan of Correction: An Ohio search of the central registry check will be conducted for Staff 1.
