Inspector Notes
A monitoring inspection was conducted on November 6, 2023. The inspector was on site from approximately 10:10 am until 12:00 pm. The director was present during the inspection. There were 23 children present, ranging in ages from three years to five years, with four staff supervising. Children were observed playing in centers that consisted of kitchen set, sensory, blocks and stencils. Small group activities included art activities and story time. The theme for the week was based around
The farm. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five
child records and four staff records were reviewed. School age children were not in care during today?s inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

1. The record for Staff #1, employed 07/31/2023, did not contain documentation of a negative tuberculosis screening, exceeding the date of employment.
2. The record for Staff #2, employed 08/23/2023, did not contain documentation of a negative tuberculosis screening, exceeding the date of employment.
3. The record for Staff #4, employed 08/10/2020, did not contain documentation of a negative tuberculosis screening, exceeding the date of employment.
4. Tuberculosis screenings are required to be submitted at the time of employment and prior to coming into contact with children as well as have been completed no more than 30 calendar days prior to beginning employment.
