Inspector Notes
An unannounced inspection was initiated on July 14, 2025, and completed on July 15, 2025, in response to a complaint received by the licensing office on June 30, 2025, relating to allegations of staffing and supervision, administration, and special care provisions and emergencies. The inspector reviewed five staff records on-site. This inspection included document review, a tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
