Each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department. The immunization reports for child #2 and child #3 were not signed by a physician or his designee

Standard 8VAC20-780-160-C
At least every two years from the date of the initial screening or testing, or more frequently if recommended by a licensed physician or the local health department, staff members and individuals from independent contractors shall obtain and submit the results of a follow-up tuberculosis screening. The record of staff #1 contained documentation of a tuberculosis screening that expired three months and 26 days ago. A follow-up tuberculosis screening had not been completed.
Plan of Correction: Per the administrator: Teacher will complete screening.

After the first license, annual approval from the health department shall be provided, or approvals of a plan of correction, for meeting requirements for water supply, sewage disposal system, and food service, if applicable. The most recent environmental sanitation report was dated 04/03/2024. An annual updated report of Environmental sanitation was due eight months ago.
Plan of Correction: Per administrator: I will call King George Health Department and arrange for a sanitation inspection.
