Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.

A monitoring inspection was conducted on 2/16/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 24 children present with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence: Child #1 (Start Date: 1/19/2022) does not have immunizations on file.
Plan of Correction: Child's immunizations and health exam will be collected and put in her file when collected from physician.
