Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/16/2024 and completed on 10/16/2024. The on-site inspection began at 10:15AM and ended at 11:30AM. The inspector reviewed compliance in the areas listed above. There were 33 children present and 7 staff. The inspector reviewed 5 children?s records and 3 staff records. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/24/24. A POC submitted after this date will not appear on the public website.

The Center is required to obtain a national fingerprint check prior to employment.

The fingerprints results for Staff #1, were dated 4 days after date of hire. The fingerprints for Staff #2, were dated approximately 6 weeks after date of hire. The fingerprints for Staff #3, were dated approximately 4 days after date of hire.

The center must request a search of the central registry prior to the employee's first day of employment.

Staff #2, who has been working approximately 3 months, requested the central registry approximately 3 weeks after their hire date. The results have not been obtained as of today, and no follow up was conducted.
