Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 03/31/2025 and completed on 03/31/2025. The on-site inspection began at 10:20AM and ended at 11:50AM. The inspector reviewed compliance in the areas listed above. There were 36 children present and 6 staff. The inspector reviewed 3 children?s records and 3 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations.

Standard 8VAC20-780-160-C
At least every two years from the date of the first initial screening or testing, staff members and individuals from independent contractors shall obtain and submit the results of a follow-up tuberculosis screening as stated in subsection B of this section.

The follow up tuberculosis screening for Staff A was not conducted two years from the previous testing. It was overdo by 17 days.
Plan of Correction: The staff member will complete the updated TB test by the end of the week.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information: Documentation of viewing proof of the child's identity and age.

Child A did not have completed documentation of viewing proof of the child's identity and age.
Plan of Correction: We will email the parent today to get the information right away.
